Job summary

Main Street Health is seeking a Health Navigator to support rural clinics by assisting patients and providers. You will develop trusted relationships with patients and providers and help with administrative and logistical tasks to allow doctors to focus on care.

You will visit clinics, meet with patients, enter data, coordinate appointments, and guide patients through insurance and resource options with an emphasis on compassionate service.

Qualifications

  • GED or high school diploma required.
  • Active unencumbered driver’s license.
  • Independent worker with strong relationship skills.
  • Service-oriented mindset and community focus.
  • Comfort using data to inform decisions.
  • Willingness to work a 40-hour week, Mon–Fri 8am–5pm.

Responsibilities

  • Develop strong relationships with patients to assist their care.
  • Enter data into the Main Street software platform for reporting and tracking.
  • Call patients to schedule visits or remind them to pick up medications.
  • Order and schedule various procedures, tests and screenings.
  • Identify and track gaps in patient care.
  • Maintain records of patient interactions and communicate with providers via EHR.
  • Educate patients on healthcare options and insurance benefits.
  • Help patients access community resources and coordinate follow-up care.

About the Role:

Navigating the changes to our healthcare system can be intimidating and confusing for doctors and patients alike. The Health Navigator’s role is to make it easier. You will start by developing trusted relationships with our patients and providers and will assist with various patient-facing, administrative, and logistical tasks so that doctors can focus on delivering high-quality medical care to their patients. Navigators work for Main Street but go to work every day inside one of our partner clinics alongside clinic staff. They meet with and call patients, enter data into our systems, contact the pharmacy to refill prescriptions, troubleshoot challenges with insurance coverage and more. Main Street’s management and training teams will equip you with the training and tools you need to perform these duties.

Responsibilities:
  • Develop strong relationships with patients to assist them with their care
  • Meet with patients during the check-in process, entering data into the Main Street software platform for reporting and tracking purposes
  • Call patients to schedule an office visit or remind them to pick up their medication
  • Order and schedule various procedures, tests and screenings
  • Identify and track progress on important gaps in patient care
  • Maintain a record of patient interactions and communicate with providers using electronic health records
  • Educate patients on their healthcare options, insurance benefits,and common medicalconditions
  • Help patients access various community resources
  • Help coordinate follow-up care after patients have been discharged from the hospital
  • Demonstrate compassion for patients, adaptability, attention to detail, patience and an eagerness to collaborate with team members
