Job summary

Qualifications

• Bachelor's degree in social work, psychology, nursing, rehabilitation, education or related field; or NYS teacher certification; or RN with bachelor's; or bachelor's and five years with behavioral health.
• Two years of experience providing direct services to persons with Serious Mental Illness, developmental disabilities or substance use disorders; or linking to services.
• Master's degree may substitute for one year of experience.

Responsibilities

• Evaluate transition from adult home to the community.
• Educate the class member on benefits of transitioning.
• Refer to Housing Contractor for in-reach when transitioning.
• Enroll in Health Home if not transitioning; sign Health Home Consent Form.
• Complete the Discharge Planning Tool (DTP) and coordinate with the CDC.
