The Health Educator works independently and as part of a team to support internal and external community health programs.

Main Duties & Responsibilities

• Conduct educational appointments with patients, prepare for and facilitate group sessions, provide patient follow-up, and present educational workshops that focus on nutrition, diabetes, disease prevention, and physical activity.
• Provide training and assistance for staff regarding health education issues.
• Act as a member of a patient care team with clinical staff and other health care providers.
• Communicate effectively with patients, health care providers, and community members.

Qualifications

Education

• Associate’s degree in Community Health or Health Education (or equivalent education and directly related experience).

Skills And Knowledge

• One or more years working in a community health setting preferred.
• Experience conducting individual or group education presentations.
• Strong interpersonal and oral communication skills.
• Ability to communicate effectively in both English and Spanish is required (may be required to assist or communicate with non-English/Spanish speakers).
• Demonstrated cultural competency to work in a low income, minority environment preferred.
