Job summary

Meeting Street is currently seeking a Health Coordinator for 30 hours per week to manage health records, conduct home visits and ensure compliance with Early Head Start standards.

The ideal candidate will have a health-related degree, LPN or higher preferred, and experience with infants, toddlers and families. The role involves community outreach, health education and coordination of care in the home or community setting.

Qualifications

  • Bachelor's degree in health-related field required.
  • LPN or higher preferred.
  • A minimum of 1 year of experience working with infants, toddlers and families in an Early Childhood setting.
  • Home visiting and or case management experience within a multidisciplinary team preferred.
  • CPR and first aid certifications or be willing and able to become certified.
  • Employees must have access to their own vehicle, with valid insurance and a valid driver’s license.
  • Meeting Street will provide an agency cell phone to maintain communication with clients and other Meeting Street employees.

Responsibilities

  • Maintain accurate and up-to-date health records for all enrolled children and prenatal clients, ensuring confidentiality and compliance with program standards.
  • Conduct home visits to assess and monitor the health, growth, and development of enrolled children and prenatal clients.
  • Ensure health services meet or exceed federal Early Head Start Program Performance Standards.
  • Enter, monitor and track health data in systems such as KidsNet and ChildPlus.
  • Provide health education to families, focusing on preventative care and addressing specific health concerns.
  • Make referrals to healthcare providers and community resources as needed and follow up to ensure services are received.
  • Conduct outreach to community organizations to strengthen program partnerships and support families in accessing healthcare services.
  • Assist with the preparation of required reports, including the Program Information Report (PIR).
  • Participate in case conferences and multidisciplinary team meetings to coordinate care for families.

Requirements

  • Bachelor's degree in health-related field required
  • Licensed Practical Nurse (LPN) or higher preferred
  • A minimum of 1 year of experience working with infants, toddlers and families in an Early Childhood setting.

  • CPR and first aid certifications or be willing and able to become certified.
  • Early Head Start health services are provided in the child’s home, natural environment, or community if applicable.
  • Employees must have access to their own vehicle, with valid insurance and a valid driver’s license.
  • Meeting Street will provide an agency cell phone to maintain communication with clients and other Meeting Street employees.
  • Ability to sit, stand, bend, walk and climb stairs.
