Job summary

Flourish Health is looking for a seasoned Head of Patient Guides to scale a high-performing team of Directors of Patient Guides across bundles of carepods. You will drive patient engagement, clinical leadership, and scalable operations in a hybrid, remote-first environment.

You will manage multi-state teams, oversee succession and change management, and ensure fidelity to Flourish’s integrated care model while maintaining a supportive culture. The role includes direct patient care hours weekly.

Qualifications

• 10+ years of relevant patient coaching related experience.
• 10+ years of people management for full time direct reports.
• 2+ years experience leading multi-state behavioral health teams.
• 2+ years experience managing through layers of leadership.
• 8+ years of startup experience.
• Able to work one evening per week.
• Must be 18 years or older.

Responsibilities

• Oversee patient guide vertical for up to 10 bundles of carepods, up to 4,500 high acuity patients.
• Each bundle has up to 15 carepods; target mature carepods with patient panel of 25–32+ engaged patients.
• Recruit, onboard, and manage Directors of Patient Guides with up to 15 direct reports each.
• Build systems for consistency, accountability, and clinical leadership across teams.
• Coach Directors on supervision, people management, productivity, and change management.
• Support leadership coverage during PTO, leaves, turnover, and interim tasks.
• Ensure high-quality patient care and fidelity to the Flourish care model.
• Collaborate across psychiatry, therapy, family guides, operations, recruiting, and people operations.
• Drive change management and culture with engagement during transitions.
• Provide direct patient care 2–5 hours per week as a leadership principle.
