Qualifications

• BSN required.
• Current Iowa nursing license by date of hire.
• Reasonable nursing experience within last 1–3 years.
• Demonstrated clinical expertise in specified practice area.
• Experience in patient education is required.
• Excellent written and verbal communication skills.
• Strong organizational, critical thinking, and problem-solving abilities.
• Experience working with an interdisciplinary team.

Responsibilities

• Collaborates with nursing, medical staff and other health professionals in planning, implementing and evaluating patient care for the specified population.
• Coordinates and facilitates patient care between UI Health Care and community services; ensures continuity of care.
• Provides direct nursing care when needed and adheres to universal precautions.
• Maintains professional standards and confidentiality; promotes a positive work environment.
• Participates in discharge planning and patient education; coordinates with families and care teams.
