Job summary

Chrysalis is seeking staff in Utah to support at-risk adults and/or juveniles with developmental and intellectual disabilities. The role requires working in stress-filled environments, following pre-approved programs, and making crucial care decisions with clear communication to the team.

Responsibilities include teaching daily living skills, planning activities, ensuring safety and cleanliness, and documenting all care needs per Chrysalis and state guidelines.

Qualifications

• Minimum of at least 18 years old.
• Pass and maintain a passable criminal background check and LEIE per state regulations.
• Pass required controlled substance tests.
• Must be able to speak, read and write effectively in English.

• Be punctual with regular attendance at the worksite.
• Stay awake, responsive, and communicate effectively with individuals.
• Teach daily living skills to individuals.
• Plan appropriate activities for individuals.
• Create opportunities for people to increase quality of life.
• Provide a clean, healthy, and safe environment.
• Implement Person Centered Support Plans and follow Behavioral Plans.
• Supervise individuals per guidelines and support their choices.
• Assist and supervise with taking the appropriate medication.
• Complete administrative tasks within established time frames.
• Complete all state and Chrysalis documentation.
• Maintain communication with the Manager regarding needs.
• Follow the Provider Code of Conduct and abuse prevention policies.
• Perform other duties as assigned by the supervisor.

Hourly wage: $15.00 - $17.75
Position Summary: Chrysalis is a company that provides a variety of support services for at-risk adults and/or juveniles that have developmental and intellectual disabilities. These individuals may also have a variety of fragile medical issues.This position requires the ability to work in stress-filled and safety-sensitive environments, to follow pre-approved programs, to make crucial decisions in regards to the care and treatment of at-risk individuals, and to communicate those decisions effectively to others. This Position helps our individuals gain greater independence by making a difference daily in applying the five Chrysalis values: Respect, Safety, Fun, Mentoring, and Accountability.

• Punctuality and regular/consistent attendance at the worksite are required and the employee must be able adhere to their workplace schedule in order to ensure proper oversight of individuals.
• Stay awake, responsive, and communicate effectively to the individuals
• Teach individuals daily living skills
• Plan appropriate activities for the individuals

• Provide a clean, healthy, and safe environment

• Maintain communication with the Manager regarding the individual’s needs
• Follow the Provider Code of Conduct/ Abuse Neglect and Exploitation Policy according to state standards
• Other duties as assigned by the supervisor

Requirements
