Responsibilities

• Assess and monitor the health status of program participants on an ongoing basis.
• Interact with program participants to better understand their needs and preferences.
• Write quarterly and annual health care summaries and submit to Program Supervisor.
• Train staff and program participants in health-related issues.
• Check medication logs and cabinets regularly, acting on any errors.
• Review and initial all incoming medical/health information.
• Administer practical portions of the med certification program.
• Review medical records and take appropriate action for any errors.
• Assist in developing and monitoring self-administration of meds program.
