Responsibilities

• Coordinate TN-HHN quality improvement and practice transformation activities across sites.
• Coordinate communications, scheduling, materials, and documentation for clinic engagement.
• Support engagement of primary care practices, health systems, and patient partners.
• Track project deliverables, milestones, and timelines to ensure grant compliance.
• Maintain protocols, workflows, surveys, and implementation tools.
• Support evaluation and learning activities related to TN-HHN initiatives.
• Engage with community organizations and patient groups for collaboration.
• Collect, organize, and track project data for reports and quality improvement.
• Assist in grant writing and reporting documentation.
