Job summary

Autumn View Health Care Facility in Hamburg, NY, is seeking a Graduate Practical Nurse (GPN) to join its nursing team. You will work under the supervision of a Registered Nurse to provide compassionate care to residents and gain hands-on experience in a skilled nursing environment.

The position pays $25.54 per hour and offers all shifts. Eligible candidates are graduates of practical nursing and will pursue licensure in New York State, with opportunities for professional growth and collaboration

Qualifications

• Graduate Practical Nurse eligible for NYS licensure.
• Strong written and verbal communication.
• Ability to follow written and verbal instructions.
• Compassionate, resident-focused approach to care.
• Ability to collaborate with residents, families, physicians, and staff.
• Strong organizational and documentation skills.
• Ability to perform frequent physical activity and assist with resident care.

Responsibilities

• Provide nursing care under RN supervision, including medication administration.
• Develop and update resident care plans.
• Monitor resident conditions and report changes promptly.
• Provide personal care, including hygiene, mobility, and feeding support.
• Document care, treatments, and observations accurately.
• Maintain a safe, resident-centered environment.
• Collaborate with interdisciplinary team to support quality care.
• Follow policies, infection control, and safety procedures.
• Participate in team meetings and ongoing education.
• Perform additional duties as assigned.
