Qualifications

  • Graduation from an accredited nursing program.
  • Active Registered Nurse license from the State of Michigan.
  • Valid driver’s license.
  • Knowledge of current nursing practices and methods.
  • At least one (1) year experience in job related field.
  • Demonstration of ability to use Microsoft Office applications and Windows 10 programs.

Responsibilities

  • Complete and document comprehensive nursing assessments of persons served.
  • Maintain accurate and timely records of persons served.
  • Develop goals and objectives with persons served/family/support system and/or guardian, and other team members for appropriate plan.
  • Provide direct nursing services to persons served.
  • Support the Person Centered Planning process.
  • Coordinate with physicians within the community.
  • Provide training/education/consultation to persons served/family/support system, staff, and/or guardian to support Individual Plan of Service.
  • Maintain appropriate, accurate and timely documentation as determined by the Agency.
  • Assist in monitoring of medication effectiveness, and assist with obtaining/understanding medications.
  • Provide after hours interventions when needed.
  • Comply with agency policies and maintain confidentiality.

ESSENTIAL FUNCTIONS
  • complete and document comprehensive nursing assessment of needs of persons served
  • maintain accurate and timely records of persons served
  • develop goals and objectives in cooperation with persons served/family/support system and/or guardian, and other team members for appropriate plan
  • provide direct nursing services to persons served
  • consistently implement and follow Person Centered Planning process
  • provide observation and monitoring of nursing service/goal implementation and recommend improvement opportunities
  • coordinate/link with physicians within the community
  • provide training/education/consultation to persons served/family/support system, staff, and/or guardian to support Individual Plan of Service
  • maintain appropriate, accurate and timely documentation as determined by the Agency
  • assist in monitoring of medication effectiveness, and assisting with enabling person served to obtain/understand medications, during clinic, program or home contact
  • provide after hours interventions (urgent needs) when needed
  • comply with Transportation Policy (09-002-0010), Alcohol & Drug Testing Policy (06-001-0010) and Background Check Policy (06-001-0015), as well as supervisors/designee directives
  • maintain confidentiality

SUPERVISORY RESPONSIBILITIES

There are no supervisory responsibilities with this position. This position receives supervision from program supervisor.

MINIMUM QUALIFICATIONS
Technical Skills
Education
  • Graduation from an accredited Nursing educational program
Licensure
  • Active Registered Nurse license from the State of Michigan
  • Valid driver’s license
Experience/Skills
  • Knowledge of current nursing practices and methods
  • At least one (1) year experience in job related field
  • Demonstration of ability to use Microsoft Office applications and Windows 10 programs
Other
  • Must have access to transportation
  • If using agency vehicles, you must pass random drug and alcohol screening

  • At least three (3) years’ experience providing nursing services to mental health service recipients
  • Demonstration of proficiency and experience working on an electronic health record system
  • Knowledge of CMH system and operations
  • Lived experience with behavioral health issues
PERSONAL DEMANDS

Personal demands refer to the physical demands, such as awkward positions, heavy lifting, etc., and the mental demands, such as concentration, attention, perception, etc. While performing the duties of this job, the employee would expect light, regular physical demand, such as frequent standing or walking; close attention to detail. The employee must occasionally lift and/or move up to 25 pounds.
