Job summary

Loma Linda University Faculty Medical Group, Department of Family Medicine, Division of Geriatrics, is seeking a compassionate NP or PA to join our geriatrics team in a SNF/inpatient setting. The role includes possible PA student teaching in a collegial, faith-based environment.

You will deliver comprehensive geriatric care and work within an interdisciplinary team. The ideal candidate has CA NP/PA licensure and geriatric experience, with a focus on improving the quality of life for older adults

Qualifications

• Current Nurse Practitioner or Physician Assistant licensure in California.
• Gerontological NP/PA with extensive geriatric experience is a plus.
• Minimum of 2 years of experience in geriatric or primary care.
• Experience in a SNF setting is preferred.

Responsibilities

• Assess geriatric patients and identify medical, psychosocial, and functional needs.
• Develop individualized care plans with interdisciplinary team.
• Provide primary care services, manage chronic illnesses, and acute care as needed.
• Prescribe, monitor, and adjust medications.
• Educate patients and families on disease management and prevention.
• Coordinate with physicians, social workers, and therapists.
