Eventus WholeHealth is seeking an Advanced Practice Clinician (NP/PA) to deliver primary care in skilled nursing and senior living communities. The role centers on routine rounding, proactive chronic disease management, and coordinating care across interdisciplinary teams with facility partners.

Qualifications

• Current NP or PA licensure in the state of practice.
• Professional patient care experience.
• Experience in geriatrics, family medicine, or internal medicine preferred.
• Exceptional patient-centered service skills, including compassionate communication, active listening, professionalism, and the ability to build trusting relationships with residents, families, and facility staff.
• Strong collaboration skills with the ability to work effectively across interdisciplinary teams.
