Job summary

Providence PACE in Oregon seeks a Nurse Practitioner or Physician Associate for full-time primary care within a geriatric and palliative-focused program. You’ll work across clinics and community settings, collaborating with physicians, social workers, pharmacists, and therapists to deliver comprehensive, person-centered care.

Ideal candidates have geriatric/post-graduate experience; training in palliative/hospice is valuable.

Qualifications

• One year of post-graduate clinical experience with geriatric population preferred.
• Training or experience in palliative care and/or hospice valuable.
• Ability to collaborate with physicians, nurses, social workers, pharmacists, and therapists.

Responsibilities

• Provide care in a variety of settings including outpatient clinics and community-based sites.
• Collaborate closely with an interdisciplinary team to deliver comprehensive care.
• Build trust with patients and families while managing complex geriatric needs.
