The Home-Based Primary Care (HBPC) physician in the Central California Health Care System provides primary health care in the patient's home and in outpatient settings, collaborating with an interdisciplinary team to improve veterans' health and well-being.

The Home-Based Primary Care (HBPC) physician practices in the outpatient setting of the Central California Health Care System and in the residence of the client. The physician is academically and clinically prepared to perform a broad spectrum of patient care services at the expert level. In addition to providing primary health care, the physician serves as a consultant to nursing and medical staff for patients with complex treatment or care needs in the home. The physician works collaboratively with other interdisciplinary team members in providing quality care and patient education, participating in research activities, and assisting patients in acute and chronic rehabilitation techniques.
Major duties include, but are not limited to: assessment, diagnosis, treatment, and outcome measures of healthcare issues; providing medical care for patients with acute and chronic health problems; delivering preventive medical care including screening and vaccinations; following up with additional home visits as appropriate; and collaborating with nursing personnel in the care of patients.
