Job Description

Colorado Physician Partners / Family Medicine Clinic is looking for an outpatient geriatric-focused internal medicine physician in the Denver metro area. CP2 is a member of the independent practice association (IPA), PHPprime, which provides wrap‑around services such as care coordination, practice transformation coaching, risk adjustment coding education, population health management, data and analytics, specialist network management, and large group contracting.

The Position

• 1.0 FTE opportunity
• Approximately 18 patients per day
• APP supervision is required
• Call schedule 1:6
• EHR – Allscripts
