Job summary

Munson Healthcare in Traverse City, Michigan is seeking a full-time Geriatrician to lead geriatric services across inpatient and SNF settings. This role offers an opportunity to shape and expand geriatric care while delivering patient-centered support within a collaborative team.

You will work with interdisciplinary teams to improve care coordination, lead clinical and operational initiatives, and mentor medical students and residents in a northern Michigan community.

Qualifications

  • MD or DO required.
  • ACGME/AOA residency in Internal Medicine or Family Medicine required.
  • Fellowship in Geriatric Medicine required or completion of training pathway.
  • Board certification or board eligibility in Geriatric Medicine.
  • Active or eligible Michigan medical licensure.

Responsibilities

  • Provide inpatient and SNF geriatric consults and follow-up as appropriate.
  • Lead and collaborate with interdisciplinary teams to enhance geriatric services.
  • Drive capacity assessments, reduce LOS, and improve quality metrics.

Thisis an exciting opportunity toshape and expand geriatric services while delivering high-quality, patient-centered careto patients across inpatient and skilled nursing facility (SNF) settings. The Geriatrician will play a key clinicaland administrative leadership role,partneringwith interdisciplinary teams to enhance geriatric consultation services, improve care coordination, and support best practices across multiplefacilities.

Job Description

Position DutiesInclude:

  • Provide medical leadership forinpatient and SNF geriatric consultative services, to include:
  • Inpatient and SNF consultations, follow-upencountersas appropriate
  • This role includes leadership responsibilities formaintaininga working relationship with SNF facilities, collaborating with SNF and hospital leaders on needs, and focusing on key metrics which include but are not limited to hospital LOS, readmissions, opportunity days, other quality metrics, etc.
  • Drivestrategy and growth into other facilities asdeemednecessary
  • Family meetings asrequired
  • Completion of capacity assessments and legal documentation asrequired
  • Participation in multidisciplinary rounds
  • Participation in clinical ethics committee
  • Participation in "length of stay rounds" with nursing managers and case management, providing medicalexpertisefor complex care management
  • Participate in and lead team huddle every weekday morning as needed
  • Collaboration with consulting teams, specialists, primary care physicians, case management, nursing, rehabphysiciansand hospice teams
  • 1.0 FTE physician must provide 36 patient contact hours, 4 hours of admin time, schedule will be set 1 month in advance
  • For Munson recognized holidays, iftheyfall on a weekday, the holiday will be coveredperschedule,an alternateoff daywill be granted to make up for holiday coverage.
  • Provideappropriate signout to team members when going off duty, so to ensure continuity of care
  • Collaborate with the Chief Medical Officer,Executive Medical Director of Medical Specialties, and Service Line Executive Directorto support the needs of the hospital and health system
  • Provide medical leadership to the geriatric team and leadershiptohelpcreate and meet operational quality and growth goals and routine measures
  • Maintain collaborative agreement with associated APPs and provide OPPE/chart review as needed
  • Provide community and multidisciplinary educationregardinggeriatric medicine including end of life decision making, management of geriatric syndromes and symptom management emphasizing pain control
  • Provide education and direction to medical students and residents that may be on service
  • Participatein regional/community events in collaboration with public relations department including participation in regional committees that wouldfacilitatethe dissemination of education throughout the community
  • Provide geriatric medicine education to primary care and inpatient providers and serve as a resource for these providers for questionsregardingspecific careplan
  • Participate in monthly geriatric practice council meetings
  • Provide oncall services for patients as isreasonably requestedbythe HOSPITAL(currently noafter hoursnorweekend coveragerequired).

  • Maintain a working knowledge of applicable Federal,Stateand local laws and regulations, HOSPITALS corporate compliance Program, Code of Ethics, as well as other policies and procedures to ensure adherence in a manner that reflects honest,ethicaland professional behavior
  • Maintain CME, board certification,licensureand credentialing through the medical staff
  • Participate in quality improvement activities asappropriateand as assigned and report results on a routine basis toappropriate medicalquality improvement programs
  • Participate with planning and implementing operational decisions,policiesand proceduresrelativeto geriatric medicine
  • Cooperate with HOSPITAL in marketing geriatric medicine services
  • Provide inputregardingequipment needs, providers and standardsrequiredand otherappropriate processease, and communicate these to appropriate administrative staff
  • Actively work with HOSPITAL to ensure patient flow and access, monitoring and controlling cost, inutilizationof services
  • Participatewithmanagement, planning and implementing programs to enhance services
