Job summary

Victory Housing, Inc. is seeking a Wellness Coordinator to implement, coordinate, and supervise health, wellness and clinical programs for Assisted Living residents.

You will work under the Delegating RN and ED to ensure proactive, holistic care and compliance with state and federal standards. The role requires Maryland licensure as a registered CMT and at least 5 years of geriatric experience, with a preferred Bachelor’s degree in health services.

Qualifications

• Must hold a current Maryland license as a registered CMT.
• Minimum of 5 years’ experience in geriatrics or related field.
• Bachelor’s degree in health services preferred.

Responsibilities

• Oversee health, wellness and clinical programs for residents in accordance with regulations and standards.
• Coordinate wellness assessments, care planning, and quality assurance with the Delegating RN and ED.
• Lead weekly resident health check-ups and monitor medications, vitals, and nutrition as needed.
• Collaborate with external vendors (podiatry, lab, home health, hospice) and internal staff to ensure holistic care.
• Participate in care planning, QA meetings, and staff trainings; serve as resident advocate and case manager.
