EmpowerMe Wellness seeks therapists to deliver therapy and wellness services to older adults within the EmpowerMe 360 model. You will conduct assessments, develop personalized care plans, and provide evidence-based interventions as part of a collaborative team.

You will support care coordination through wellness check-ins, caregiver communication, and telehealth. The role emphasizes maintaining professional standards and contributing to ongoing program development and quality improvement.

Qualifications

• Current professional license in PT, PTA, OT, COTA, or SLP.
• Passion for empowering seniors to live their best lives.
