Job summary

San Antonio North Nursing and Rehabilitation seeks a Social Service professional to plan, develop, and direct the facility's social service program to meet residents' medically related social and emotional needs.

The role supports the Social Service Director, collaborates with administration and clinical staff, ensures policy compliance, documents progress, and assists residents and families with Medicare/Medicaid resources.

Qualifications

• Bachelor’s Degree in Social Work (BSW) required.
• One year of supervised social work experience in a health care setting.
• Social work licensure (LSW/LMSW/LCSW) or state equivalent if required by state law.
• Must meet all state and federal qualification standards for social workers in skilled nursing facilities.
• Completion of mandatory in-service trainings, including HIPAA.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Provide information to resident/families about Medicare/Medicaid and other financial assistance.
• Record and maintain social service progress notes and coordinate with other departments.
• Assist with discharge planning and coordination with community agencies.
