Job summary

OPCO Skilled Management is seeking a Social Worker to identify psychosocial and emotional needs, develop access to services, and ensure care aligns with NASW ethics and regulatory requirements in a skilled nursing facility.

The role involves completing assessments, maintaining thorough documentation, supporting resident autonomy, and coordinating discharge planning with the interdisciplinary team. Strong collaboration with staff and families is essential.

Qualifications

• Licensed, certified, or registered in the state of practice is required.
• Bachelor’s Degree in Social Work or Human Services field from an accredited school is required.

Responsibilities

• Completes social history and psychosocial assessment for each resident.
• Utilizes the minimum data set (MDS) to guide the care plan.
• Maintains documentation per federal/state regulations and center policy.
• Supports resident autonomy and documents refusals and alternatives.
• Develops an interdisciplinary plan of care addressing psychosocial needs.
• Coordinates discharge planning and referrals to home care services.
• Collaborates with the care team to protect resident rights and well-being.
• Assists in education and support for residents and families.
• Participates in survey readiness and maintains compliance.

Education

Bachelor’s Degree in Social Work or Human Services field
