Job summary

Requirements: Bachelor’s in Social Work or Human Services field, 1 year of supervised healthcare social work with geriatrics; 2 years nursing in a Skilled Nursing Facility

Qualifications

• Licensed or registered in Texas; LBSW preferred.
• Bachelor’s Degree in Social Work or Human Services field required.
• Minimum 1 year of supervised social work in healthcare with geriatric residents.
• Minimum 2 years nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social histories and psychosocial assessments to identify needs.
• Maintains documentation compliant with federal/state regulations and center practices.
• Supports resident self-determination and educates on therapeutic alternatives.
• Develops interdisciplinary care plans addressing psychosocial needs and goals.
• Coordinates discharge planning and referrals for home care services.
• Collaborates with nursing and admin to protect resident rights and well-being.
• Participates in education for residents, families, and staff about facility issues.
