Qualifications

• Previous experience as a Social Worker Assistant preferred.
• Valid Social Worker License.
• Two years experience working directly with geriatric clients in a health care setting, desirable.
• MSW/CSW preferred.
• Customer service, in-person and over the telephone.
• Some knowledge of medical insurance, Medicare/Medicaid program benefits, desirable.
• Discharge planning.
• Computer literacy, able to fax, scan, email.
• Experience in clinical patient conferences.
