Qualifications

• Bachelor's degree in Social Work or related field.

• Ability to maintain confidentiality and advocate for residents.

Responsibilities

• Complete psychosocial assessments, care plans, progress notes, and other required documentation.
• Provide counseling, emotional support, and guidance to residents and families.
• Coordinate discharge planning and transitions of care.
• Advocate for resident rights, dignity, privacy, independence, and quality of life.
• Collaborate with nursing, therapy, activities, dietary, and other departments.
