Providence Living Center in Topeka, KS is seeking a Social Services Designee to join our team. Qualifications

• High school diploma required.
• SSD certificate or willingness to obtain.
• Ability to be physically present onsite at the community.
• Good listening skills.
• Proficient with Word and Excel, email; intermediate computer skills.
• Ability to maintain confidentiality and comply with HIPAA regulations.
• Desire to work with the elderly.

Responsibilities

• Develop a social history, social assessment and care plan for residents.
• Document progress notes related to each resident's plan of care.
• Implement staff approaches addressing problems and review effectiveness.
• Participate in interdisciplinary Care Plan meetings.
• Assist in discharge planning and placement for residents.
• Organize family groups to promote communication and provide counseling as needed.
