Responsibilities

• Assess referred patients and develop individualized rehabilitation plans.
• Document patient condition, goals, progress and discharge plans.
• Coordinate with physicians and other disciplines for patient-centered care.
• Supervise Occupational Therapy Assistants and ensure quality care.
