Job summary

Creekside Center for Rehabilitation and Healing is seeking a full-time Physical Therapy Assistant to join our skilled nursing facility team. The ideal candidate is passionate about geriatric care, licensed in the state of practice, and able to evaluate and treat independently without direct supervision.

Assist the PT with therapies designed to improve mobility, relieve pain, and promote overall fitness and wellness in residents. Monday–Friday schedule. Creekside is an Equal Opportunity Employer.

Qualifications

• Must possess license in the state of practice.
• Able to evaluate and treat independently without direct supervision.
• SNF/geriatric setting experience preferred but not required.

Responsibilities

• Assist the Physical Therapist with therapies to improve mobility and relieve pain.
• Promote overall fitness and wellness for residents in a skilled nursing facility.
• Support discharge planning and resident functional goals.
