Renewal Rehab in Wayne, Michigan seeks a caring Physical Therapist Assistant to join our team serving seniors in skilled nursing and post-acute care. You will deliver individualized treatment plans under the direction of a Physical Therapist, focusing on improving mobility, safety and independence.

You will perform therapeutic exercises, gait and balance training, and functional activities while documenting progress in our EHR system.

Qualifications

• Certified Physical Therapist Assistant able to work under a PT direction.
• Experience delivering PT-related treatments to seniors in SNF or post-acute care.
• Proficient in documenting progress in an EHR system.
• Commitment to patient-centered, collaborative care.

Responsabilités

• Deliver individualized treatment plans under the supervision of a Physical Therapist.
• Perform therapeutic exercises, gait and balance training, and functional activities.
• Document progress and patient responses in the EHR.
• Collaborate with the PT and care team to adjust care plans.
