Western Geriatrics & Neurology is seeking a compassionate Physician Assistant or Nurse Practitioner to join our Mobile Primary Care Team, delivering medical care to older adults in homes and senior living communities across California.

You will perform comprehensive assessments, develop care plans, manage chronic conditions, and collaborate with palliative, psych, and neurology specialists to provide whole-person care.

Qualifications

• Active NP or PA license in the state of practice.
• 1–2 years clinical experience in geriatrics or primary care preferred.

• Valid driver’s license and reliable transportation.
