Job summary

Twin Cities Physicians in Rochester, MN is seeking a Physician (MD/DO) for Senior Living Rounding covering assisted living, memory care, SNF and long-term care. You will provide on-site primary care, coordinate with care teams, and guide families through complex medical decisions.

The role centers on geriatric medicine and comprehensive care planning, with a compensation package including base salary, productivity incentives, mileage reimbursement, CME support, and full benefits.

Qualifications

• MD or DO licensed in Minnesota in good standing.
• Experience in geriatrics, internal, family medicine, SNF or post-acute care preferred.
• Strong clinical judgment and ability to manage complex patients.
• Excellent communication with residents, families and staff.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide on-site primary care and rounding across living communities.
• Manage chronic disease, preventive, transitional, and post-acute care.
• Diagnostics, treatment planning, med management, and follow-up.
• Collaborate with NPs, PAs, nursing staff, therapists, and teams.
• Support residents and families in complex decisions and care planning.
• Maintain accurate EMR documentation and coordinate care.
