Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one care to older adults in private homes and senior living communities in Old Saybrook, CT. Clinicians may integrate telehealth and in-person visits, tailoring treatment plans to each patient’s goals.

Qualifications

• Licensed Physical Therapist in CT or eligible to obtain license.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home, senior living, or community settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence and function.
