Job summary

Personal Touch Home Health Care Services Inc. seeks a dedicated registered nurse to provide patient-centered nursing care for older adults in an outpatient senior primary care setting in Gainesville, Florida.

You will perform comprehensive assessments, monitor chronic conditions, manage acute changes within scope of practice, and coordinate follow-up and medication reconciliation with an interdisciplinary team to ensure continuity of care.
