Job summary

Chatham Hills Subacute Care Center seeks a licensed Occupational Therapist to join our rehabilitation team. You will assess patients, develop and implement individualized treatment plans, and document progress while educating patients and families.

Qualifications

• Must have graduated from an accredited Occupational Therapist program.
• Valid professional license and registration for state of practice.
• Overseeing and supervising Occupational Therapy Assistants.

Responsibilities

• Assess referred patients to determine rehabilitation needs.
• Establish and administer treatment programs with goals aligned to patient capacity.
• Document patient condition and progress; educate patients and families.
