Job summary

Renewal Rehab is seeking an Occupational Therapist to provide high-quality, patient-centered care to seniors in skilled nursing and post-acute care settings. You will assess functional abilities, create evidence-based treatment plans, and deliver interventions that promote independence, safety, and quality of life.

You’ll collaborate with an interdisciplinary team, maintain accurate documentation, and educate patients, families, and staff.

Qualifications

• Requires OT licensure in Illinois.
• Master's or Doctorate in Occupational Therapy preferred.
• Experience in SNF/post-acute care settings is a plus.

Responsibilities

• Assess functional abilities and develop treatment plans.
• Deliver evidence-based interventions to promote independence.
• Maintain accurate documentation and progress notes.
• Educate patients, families, and staff; collaborate with interdisciplinary team.
