Job summary

Renewal Rehab in Illinois seeks a licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed Occupational Therapist with eligibility to practice in Illinois.
• Experience delivering OT services to seniors in SNF or post-acute settings (preferred).
• Ability to develop and implement individualized treatment plans.

Responsibilities

• Evaluate residents and assess functional needs.
