Job summary

Renewal Rehab is seeking an Occupational Therapist to provide patient-centered care to seniors in skilled nursing and post-acute settings. You will assess functional abilities, develop evidence-based treatment plans, and deliver interventions that promote independence, safety, and quality of life.

You will collaborate with an interdisciplinary team, maintain accurate documentation, and educate patients, families, and staff.

Qualifications

• OT degree and state licensure required.
• Experience in SNF or post-acute care preferred.
• Ability to develop and implement evidence-based treatment plans.
