Job summary

Renewal Rehab is seeking an Occupational Therapist to provide high-quality, patient-centered care to seniors in skilled nursing and post-acute care settings. You will assess functional abilities, create evidence-based treatment plans, and deliver interventions that promote independence, safety, and quality of life.

You’ll collaborate closely with an interdisciplinary team, maintain accurate documentation, and educate patients, families, and staff.

Qualifications

• Licensed Occupational Therapist in the United States (state licensure).
• Experience in skilled nursing or post-acute care settings preferred.
• Strong documentation and patient/family education skills.

Responsibilities

• Assess functional abilities and risks; develop evidence-based treatment plans.
• Deliver interventions that promote independence, safety and quality of life.
• Collaborate with interdisciplinary team and maintain accurate documentation.
