Renewal Rehab is seeking an Occupational Therapist to provide high-quality, patient-centered care to seniors in skilled nursing and post-acute care settings. You will assess functional abilities, create evidence-based treatment plans, and deliver interventions that promote independence, safety, and quality of life.

You’ll collaborate closely with an interdisciplinary team, maintain accurate documentation, and educate patients, families, and staff.

Responsabilités

• Assess functional abilities and mobility to inform treatment planning.
• Create evidence-based treatment plans tailored to seniors in SNF and post-acute settings.
• Deliver interventions that promote independence, safety, and quality of life.
• Collaborate closely with an interdisciplinary team and maintain accurate documentation.
• Educate patients, families, and staff on goals, progress, and safety.
