Renewal Rehab is seeking an Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute settings. You will assess abilities, create treatment plans, and deliver interventions to promote independence and safety.

Collaboration with an interdisciplinary team, along with mentorship and opportunities for professional growth, are part of the culture. You will maintain accurate documentation, educate patients and families, and contribute to meaningful outcomes

Formación

• Licensed OT in Illinois or eligible for license
• Experience in SNF or post-acute care settings preferred
• Ability to develop evidence-based treatment plans

Responsabilidades

• Assess functional abilities of seniors in SNF/post-acute care
• Develop and implement evidence-based treatment plans
• Deliver interventions to promote independence and safety
• Educate patients, families, and staff; document accurately
