Qualifications

• Excellent verbal and written communication and organizational skills.
• Ability to read, analyze, and interpret medical information and communicate with residents, families and healthcare professionals.
• Proficiency with Word and Excel.

Responsibilities

• Evaluate and document resident evaluations per facility policies.
• Provide treatment to help residents achieve the highest possible function.
• Recommend adaptive equipment and preventative measures to maintain function.
• Educate residents and caregivers to support independence in activities of daily living.
• Maintain timely resident reports for regulatory agencies and third-party payers.
• Participate in care conferences and interdisciplinary meetings.
