Job summary

Greenhills Health and Rehabilitation Center is looking for a talented and hard-working Occupational Therapist to join our ever-growing team.

The Occupational Therapist will assess patients requiring rehabilitation, establish and administer treatment plans, and set goals based on capacity. You will document patient progress and educate patients and families while collaborating with a multi-disciplinary team to deliver patient-centered care.

Qualifications

• Must have graduated from an accredited Occupational Therapist college or university.
• Must have a valid professional license and registration for state of practice.
• The Occupational Therapist is responsible for overseeing and supervising Occupational Therapy Assistants.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence
• Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.

Responsibilities

• Assess referred patients to determine rehabilitation needs and goals.
• Develop and administer treatment programs aligned with patient capacity and tolerance.
• Document patient conditions and progress; educate patients and families.
• Supervise and oversee Occupational Therapy Assistants as needed.
• Collaborate with interdisciplinary team to ensure comprehensive care.
