Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. In this onsite role, you will evaluate residents, develop individualized treatment plans, and deliver evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed Occupational Therapist with active IL license.
• Experience in geriatrics or SNF/post-acute settings preferred.

Responsibilities

• Evaluate residents in skilled nursing facility/post-acute settings.
• Develop individualized OT treatment plans.
• Deliver evidence-based interventions to improve ADLs, mobility, cognition and safety.
• Document in EMR and maintain accurate patient records.
• Collaborate with an interdisciplinary team and communicate progress.
• Educate families and caregivers on care plans and safety practices.
