Job summary

Renewal Rehab is seeking a Licensed Occupational Therapist to provide patient-centered care for seniors in skilled nursing and post-acute facilities. This onsite role involves evaluating residents, developing individualized treatment plans, and delivering evidence-based interventions to improve ADLs, mobility, cognition, and safety.

You’ll document in EMR, collaborate with an interdisciplinary team, and educate families and caregivers.

Qualifications

• Licensed Occupational Therapist in Illinois.
• Experience delivering elder care in SNF or post-acute settings preferred.
• Ability to assess, plan, and deliver ADL-focused interventions.

Responsibilities
