Job summary

Renewal Rehab is seeking an Occupational Therapist to provide high-quality, patient-centered care to seniors in skilled nursing and post-acute care settings. You will assess functional abilities, create evidence-based treatment plans, and deliver interventions that promote independence, safety, and quality of life.

You’ll collaborate closely with an interdisciplinary team, maintain accurate documentation, and educate patients, families, and staff.

Qualifications

• Licensed OT in Illinois or ability to obtain licensure.
• Experience delivering patient-centered care to seniors preferred.
• Ability to educate patients, families and staff.

Responsibilities

• Assess functional abilities of seniors.
