Job summary

TruHealth is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with experience in primary care or internal medicine, preferably geriatrics. The role focuses on improving long-term care outcomes by collaborating with a team of providers and case managers.

You’ll work in nursing homes or assisted living facilities, providing personalized care, prescribing medications, and coordinating tests while implementing Treat in Place protocols in compliance

Qualifications

• 3 years’ experience in clinical nursing or rehab in geriatric populations
• APRN or PA license required
• Electronic Health Records experience

Responsibilities

• Assess patients’ medical and mental health needs and create care plans
• Prescribe medications and order labs and consultations
• Monitor treatment and adjust plans as needed
• Ensure compliance with local, state, and federal regulations
