Optimal Care in Carmel, IN is seeking a Nurse Practitioner or Physician Assistant to provide primary and consultative care primarily for geriatric patients. Responsibilities include conducting medical evaluations, developing treatment plans, and ensuring continuity of care. The ideal candidate will hold a Master’s degree as a Board-Certified Nurse Practitioner or Physician Assistant, have a current state license, and demonstrate the ability to assess and respond to patient needs. Competitive pay range of $100,000 — $120,000 USD is offered.

Qualifications

• Master's or doctoral degree as a Board-Certified Nurse Practitioner or Physician Assistant.
• Current State License as a Nurse Practitioner or Physician Assistant.
• Ability to sit, stand, bend, lift, and move intermittently.
• Reliable transportation and valid automobile insurance coverage.
• Demonstrates ability to assess and respond to the needs of patients.

Responsibilities

• Conducting medical evaluations of elderly, frail, seriously ill patients.
• Communicating findings and changes in condition to the care team.
• Performing a physical exam based on age and patient history.
• Developing treatment plans in collaboration with the patient.
