Job summary

We Care Health Centers in Ohio seeks a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities. You will work within an interdisciplinary team to manage acute changes in condition and implement care plans.

Responsibilities include prescribing meds per OH law, coordinating with nursing home staff, ordering tests, and collaborating with primary care physicians. OH license and DEA required; board certification preferred.

Qualifications

• Current, unrestricted NP license in the state of OH.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus

Responsibilities

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.
