Job summary

1 Care Partners is seeking a compassionate Nurse Practitioner (NP) to deliver high-quality healthcare in a Skilled Nursing Facility (SNF) in Amherst, VA. The position offers a competitive salary plus benefits and both full-time and part-time options.

The NP will collaborate with physicians and the care team to assess, diagnose, and manage geriatric patients, develop treatment plans, and provide education to patients and staff. A Virginia NP license and SNF/geriatric experience are preferred.

Qualifications

• Active Nurse Practitioner license in Virginia.
• Experience in SNF/geriatric setting preferred.
• Strong clinical assessment and decision‑making skills.
• Ability to work independently while collaborating with a multidisciplinary team.
• Excellent communication and interpersonal skills.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Diagnose and manage acute and chronic conditions in coordination with physicians.
• Educate patients, families, and facility staff on disease management and preventive care.
• Maintain accurate and timely documentation in compliance with regulatory guidelines.
• Collaborate with interdisciplinary teams to ensure quality patient outcomes.
