Job summary

OnSite Medical Solutions in Charleston, SC seeks a Nurse Practitioner to provide consultations across nursing homes and assisted living facilities, delivering high-level medical and mental health care with administrative support and clinical mentorship. The candidate should hold MSN with FNP or related certification, active SC license, DEA registration, and at least one year of NP experience.

Qualifications

• MSN with FNP, ANP, AGNP, AGACNP, AGPCNP, or ACNP certification.
• Active License in South Carolina.
• DEA registration.
• 1+ year NP experience.
• Excellent communication and documentation abilities.
• Monday–Friday visiting schedule with flexibility.

Responsibilities

• Round at assigned facilities and manage acute, subacute, and primary care needs.
• Collaborate with OnSite team, nursing home staff, residents and families.
• Proactively follow up with patients and assess intervention efficacy.
• Provide education on best practices.
