Responsibilities

• Evaluate clients to understand needs and mental health conditions.
• Create individualized care plans for each client.
• Collaborate with healthcare professionals, nursing staff and case managers to coordinate services.
• Monitor clients’ progress and adjust care plans as needed.
• Maintain accurate and confidential records of treatments and interventions.
• Provide talk therapy using evidence-based techniques.
